Add a trip request form to your website

Let customers request transportation from your company. Relay receives each request and sends it to your transport provider’s TPP queue for dispatch review.

Requests remain pending office review. Dispatch must confirm availability, timing and payment arrangements before a ride is arranged.

Set up your transport provider form

An authorized Relay administrator links your form to your TPP transport provider identity and configures your display name, contact phone, active status and approved website domains.

Supply every hostname where you will embed the form, including www and your staging hostname when used. Only approved websites can display it in a frame.

Open Relay form configuration (Relay administrator access required). Transport provider fleet and operating settings remain in TPP Setup / Settings.

Copy your link or embed code

Select your company below. If it is missing, ask your authorized Relay administrator to configure or activate its request form.

Choose your company to get its existing Relay form link and embed code.

A separate link is also useful for a “Request a ride” button. Each transport provider gets the same Relay form design with its own server-bound destination.

Add it to a draft website page

Paste the complete embed code into your website’s HTML or Embed block. Keep the “Open form separately” link beneath the frame.

WordPress

Add a Custom HTML block, paste the code, and preview the draft. Your WordPress role must allow iframes. If your editor removes the iframe, ask your website administrator to add it using the site’s approved embed method.

Other website builders

Use an Embed or HTML section. Let the frame use the full content width and keep its height large enough for the review step. Keep vertical scrolling available. If your plan does not allow iframes, use the separate form link on a button.

Embed the Relay URL as supplied. Do not rebuild the form in your website builder or add passenger fields to a marketing form.

Test the full request

  • Open the draft on a phone and desktop; check all four steps, Back/Edit, scrolling and keyboard navigation.
  • Review the requested details and submit once. The receipt must say dispatch needs to confirm the ride.
  • Verify the pending request in Relay intake and your transport provider’s TPP Trip requests queue.
  • Check that a retry keeps one reference, and that the separate form link and configured phone number work.

Rider details go directly to Relay. Your website should not capture them in analytics, emails or its own database. A public form link does not provide access to existing requests or staff functions.

If the form does not load

Use “Open form separately” first. Ask your Relay administrator to check the exact website domain and active status. Keep your staff portal’s security settings unchanged.

If your transport provider cannot accommodate a request, flag it for Relay review in TPP. The preferred transport provider stays attached to the request.
