This website provides information about Relay and ways to contact us or request transportation review. An inquiry, company application or ride request does not confirm a booking, payment authorization, provider approval, account access or pickup time. Dispatch must confirm transportation arrangements.

Use the appropriate form

Business inquiry and provider application forms are for business contact information. Do not include patient information, medical records, insurance documents, passwords or payment details. Use the designated ride-request process and its instructions for transportation requests.

Your communication choices

Permission to reply by phone or email is separate from optional SMS consent. See the SMS terms for the limited business-inquiry preference and the planned return-pickup program. Website forms do not enroll passengers in SMS automatically.

Privacy and maps

Read the Privacy Policy and the separate maps and routing terms. Route and travel-time estimates do not guarantee an arrival time.
